  1. Note. # Proportions do not add up to 100% because of staff changes. Of the 11 staff interviewed pre-implementation training, three had resigned (n = 1), rotated to another area (n = 1) or taken maternity leave (n = 1) by mid 2008. Of the eight staff interviewed in mid 2008, two were new employees.
