Definition of variables: Gender: male = 1, female =2. Age: 20–29 = reference, additional four variables for each age group 30–39, 40–49, 50–59, 60–69. Family income: lowest incom quintile = reference, additional four variables for each quintile
Education: Primary education (1-10th grade) = reference, additional three variables with the highest completed educational leve, secondary education (10th -14th grade), lower level university education (14th – 17th grade) and higher level university education (more than 18 years)
