  1. *The BCDRS and SF-12 items were dichotomous (yes/no). The GUSTO trial items had 4 options from None of the time, some of the time (1–2 days/wk), some of the time (3–4 days/wk), and All/most of the time (5 or more days/wk). The GUSTO items were dichotomized around the median of 3 or more days/wk.
