  1. * Denominators in brackets are the number of most recent scheduled services (within the preceding 12 months of each audit) according to guidelines, which varied at each audit depending on the age of the client and the scheduled frequency of the service. §Odds ratio (OR) represents the average changing of probability for the delivery of each service with the increase of one unit of time (1 year), adjusted for the recent attendance at health centres using two-level logistic random-effects model.
