  2. † HADS contains 14 questions and consists of two subscales: anxiety and depression. Each question is rated on a four-point scale, giving maximum scores of 21 for anxiety and depression. Scores of 0 - 7 represent normality, 8 - 10 borderline disorder and 11 or more represents a significant 'case' of psychological morbidity.
  3. * SF-36 scores are scored on a range of 0 (poorest health) - 100 (best health possible).
  4. ΔSEIQoL global score is calculated by multiplying the individual's current self rating on each cue by the corresponding cue weight and summing the products across the five cues. This global quality of life score can range from 0 to 100
