  4. Level of care: community hospitals (labeled A) having no catheterization facility, intermediary hospitals (labeled B1) providing coronary angiography but no Percutaneous Coronary Intervention (PCI), and tertiary hospitals (labeled B2-B3) offering PCI and/or Coronary Artery Bypass Graft.
